Simply install the app and perform the initial sync and you instantly have access to a vast database of known telemarketers that will all be labeled as "Do Not Answer" (or whatever other name you'd prefer). Then, any time you receive a call from a number that isn't recognized simple add it to the "Do Not Answer" contact using the iPhone's build-in addressbook and "Add to Existing Contact" button.
